INSURANCE & RATES

Some PPO insurance plans are accepted, please email me for insurance verification. Please include your name, your date of birth, your member ID number, and your insurance group.

Payment is collected at each session. Cash, check, and credit cards are accepted.

Cash Rates:
Initial Evaluation & Follow-Up Physical Therapy Sessions: $160 per session
